muy guapo means very handsome.

But guapísimo means the superlative....the most handsome....one of the most handsome....more handsome than the guy that is very handsome....it is just a greater intensifier than muy.

Los sufijos -ísima e -isimo, sirven para formar el grado superlativo de algunos adjetivos, aunque en algunos de ellos se prefiere la formación con la forma culta, a partir de los sufijos -érrimo y -érrima.
