Dear Dr. Hendrix:

I had the pleasure of examining your patients on 11/30/11. The forum presents with an acute case of sorrow and regret due to our lack of remembering your birthday last week!

Surprisingly, the heart sounds were normal. There were no murmurs, gallops or rubs. No cardiac bruits were noted The remainder of the exam was within normal limits.

I recommend an MRI of our brains along with a healthy dose of good wishes for you in the coming year p.r.n.. A follow up appointment has been scheduled for November 23rd, 2012.
