Take all 3 replies and you may get one spelled and punctuated correctly.

¿Cómo se llama?

¿Cómo te llamas?

¿Cuál es tu nombre? is less common, but still correct.

Well, take at least the te and se options, depending on the age and status of the other person. - annierats, Nov 27, 2011

Or, "¿Cómo se llama?" if they are older than you or in a more senior position.

In Spain most of the time, these days, second person (te) is used.
