What does biggest mean? Largest land coverage? Largest population?
Madrid seems to be, by far, the largest by population (twice as many citizens as Barcelona , city No. 2)
As to how it is pronounced: ask the Reina madrileña here on this forum to see how She pronounces the d's. Of course, Her word is final on how it is to be pronounced. (as with all of Her pronouncements)
