How does one tell the difference between novio as in fiance and novio as in boyfriend or girlfriend?

In Bolivia I think it works like this:
Prometido / a masculine or feminine noun = fiancé, (f) fiancée
Novio /a = a serious relationship = boyfriend / girlfriend
Amigo / a -= friend.
