need help filling in spanish form 030

I need to change to my own address in Spain, instead of my Spanish solicitors address. As far as I know I need to fill in form 030, so that I can pay my own Renta tax. I know how to fill in form 210 but need help with form 030, so that I can get the (Pagatins) stickers required to put on form 210

There is a downloadable pdf instruction document for filling out the form. I downloaded it from the site mentioned in the link above. (the modelo 030 document itself is there also. Just enter modelo 030 into their search engine box).
