What does "ayúdame" mean?
is it a verb conjugation for ayudar? Why does it end with -me? Can someone please explain? And what the rule is and difference?

HI and welcome to the forum.
In Spanish we attach the pronoun to the end of a positive a command:
Ayuda + me (Command "Help" + me) = ¡Ayúdame!
Negative commands are different:
¡No me ayudes! = Don't help me!
It means- Can you help me.
When you have Spanish verb in the infinitive form, or the to form ( to play, to help, to go, etc... ) you can hook me, te, le , lo right on the end of it. So ayudarme= to help me, ayudarte= to help you, ayudarle= to help him/ her/ darme= to give me, darte= to give you, darle= to give him/her comerlo= to eat it hacerlo= to do it
Ayúdame by itself is used when telling someone to help you like a boss would say to an employee.
Ayúdame por favor or favor ayúdame is used when asking for help

You should decide on what you want to ask. Your title (What does "ayúdame" mean?) asks one question but the body of your post asks about "¿Puedes ayudarme?" (not a command).
Mariana already mentioned suffixing the pronoun to the imperative, it can also (and usually is) suffixed to the infinitive (it can also be a suffix for a "gerundio" [present participle]),

¿Puedes ayudarme? means: Can you help me?.
