Would this be the correct translation to ask where someone is going (formal)? Is it also the same as "¿Adonde va?" Can these two phrases be used interchangeably?

Yes, the're are correct. You also can say: A dónde se dirige? Hacia dónde se dirige? A dónde va? (not "adonde")

Could "A dónde se dirige" mean, where does it lead / go when referring to a path that you don't know where it leads?
