  -El maesto/profesor es de + nombre ciudad ó País 

¿Cómo se llama el maestro?

  -El maestro se llama + nombre

¿Cómo te llamas?

 - Mi nombre es + nombre

¿De dónde eres tú?

 -Yo soy de + nombre ciudad ó País

¿cómo estás?

 - Yo estoy + estado de animo

Why don't you start by trying to translate them into English. Then you'll be able to answer them in English. Then try to translate those answers into Spanish.

Those answers are something that someone here on the forum would be happy to help you with.
