Spanish has its own set of idiomatic expressions, every bit as colorful. It's not productive to attempt to translate English idioms directly as you encounter them.
If you google spanish idioms, you will find plenty of sites that will list idioms from which you can learn a lot. You may even find some that expresses your reaction to this great photo even better than "completely mental".
