Simply,
Por favor, complete este fomulario.
Oddly enough, the formal command form of "completar" and the english "to complete" are spelled identically.
You could use rellene (rellenar) instead of completar. "Fill in" instead of "complete".

Proabably good to say please or ask 'would you'.
This is in Colombia, I'm sure it's said differently elsewhere - llena, rellenar, formulario, y cosas así...
My girlfriend has just told me that formulario is more commonly used here, though I have heard formato too.

The dictionary on this site says

but I would say
completar este impeso
