Does the phrase mean "Can you help me reach this?" or "May I reach this?"

I would interpret this is as Can you reach that for me?

This is incorrect because the subject is "puede". If it were "I", it would have to be "puedo".

"Eso" isn't this, it means that. It is a neuter demonstrative!

If were asking for help, we'd need the verb "ayudar" in here somewhere. Also, remember..."eso" means "that".

"¿Puede ayudarme para alcanzar eso?" --- I'm pretty sure that's how one would say "Can you help me reach that?"

When you ask someone to "alcanzar" something for you, you are asking them to get it for you or pass it to you because you can't reach it from where you are.

"Alcánzame la sal, por favor." - "Pass the salt, please."

"¿Me puede alcanzar esa lata? Soy demasiado bajo." - "Can you get that can for me? I'm too short."
