It's not incorrect without the usted...it's just a little vague.

When using "tienes" alone, the person reading or listening will know that "tú" (you informal) is the subject.

However, when using "tiene" alone, the reader or listener will have to determine whether the subject is "él" or "ella" or "usted". It's okay to leave out the "usted" if it's been established that "you formal" are the subject.

All of the following are perfectly correct when asking if "you" have a mirror.

"¿Tienes un espejo?"

"¿Tiene un espejo?"

"¿Tiene usted un espejo?"

Remember that you informal (tú) is used for friends, someone younger than you, even close family members. You formal is used for someone older than you, someone you just met, and someone you want to show respect to.

No it's not! ¿Tiene un espejo? sounds very good to me if you were talking to an older person, someone you meet for the first time, or a person you're not familiar with.

¿Tienes un espejo?" as informal

This is correct as well, and used with people you're familiar with!
