You can say this is in many ways in Spanish:
Spoken and polite: ¿Cómo se llama Ud? = What is your name?
(This form is used when talking to people you do not know, people in authority like your boss, policemen or when talking to people older than yourself)
Spoken and informal/familiar: ¿Cómo te llamas? = What is your name?
(This familiar or friendly form is used when talking to family, friends, people you know well, and children/people younger than you)
Spoken/Written formal:
¿Cuál es su nombre? = What is your name?

If you are talking to somebody you would want to show respect to (a formal situation), you could say,
"¿Cómo se llama usted?"
If you are in an informal situation,
¿Cómo te llamas?
Another way is
