Are you asking about Spanish or English? Your profile is not filled out.

In English, that would more commonly include the word "Do". Do we need to schedule your next appointment?

I would say "Favor de hacer una cita" but if you wanted literal I would say "Necesitamos que hagas tu próxima cita". ( Tu form is doubtful there though and the whole thing sounds a bit weird to me).
