una alcantarilla obstruida [atascado] might convey the idea.

Nope, just a blocked sewer, no back-up.

cloaca=sanitary sewer/combination sewer

un alcantarillado sanitario obstruido

I am assuming that you mean a sanitary sewer? Like when your sewer backs up and floods your basement?

A blocked sewer, I would say : Una coladera tapada.
