Close, but not quite.

"Do you want..." can be "¿Quieres...?" or "¿Usted quiere..."

"Querer" is the verb meaning "to want".

"Quieres" is the tú form of the present tense, indicative mood. You don't necessarily need to say, "Tú quieres". You use the tú form when talking to someone younger than you, someone you are friends with, or someone you are very close with.

"Quiere" is the él/ella/usted form. Since the third person form can have multiple subjects, I tend to use the subject pronoun with it. You use the "usted" form when referring to someone you just met, your elders, or someone you want to show respect to.

¿Quieres una galleta? - Do you (familiar) want a cookie?

¿Usted quiere una galleta? - Do you (formal) want a cookie?
