You would have to provide a specific context, but, in general, they can.
Por eso=for that reason
por lo tanto=therefore, so

More loosely translated with the help of my Cuban buddy.
"Por eso" literally means "for that" but is used more like "because of that".
EX: *"Eso es una locura y por eso, no voy a hacerlo."
- "That's craziness and because of that, I'm not going to do it."*
"Por lo tanto" more so means "that's why (or therefore)".
EX: *"Eso es una locura y por lo tanto, no voy a hacerlo."
- "That's craziness and that's why I'm not going to do that."*

Por lo tanto means 'meanwhile' or 'therefore', while por eso means along the lines of 'that's why' (literally 'for that')
