Well someone might have to amend this on mine, but for Spanish as a primary language there is Spain, Mexico, all of South America (except Brazil).
But spanish is spoken throughout USA (particularly California), maybe brazil or portugal (because of its neighbours).

Mexico, 5 in Central America, 9 in South America, 3 in the Caribbean and Spain. That's 19. There are some countries that were Spanish colonies who still speak some Spanish but it is not the main language. For example, the Phillipines which were settled by Spaniards in the 17th century (and where Magellan was killed and eaten by natives), still uses Spanish but also uses English because Spain lost all its colonies to the USA in the Spanish American war, including the Phillipines, and their national language is Tagalog. Marruecos (Morocco) in Africa was at one time a Spanish colony, at another time a French colony, so both speak some Spanish and some French but their official language is Arabic.
So the answer is 19.
There are Spanish speaking countries around the world on almost every continent. Did you know there are two independent cities in Africa that speak Spanish? Or that until a few years ago, the island of visaya in the Philipines used Spanish as their primary language? I no longer have the figure, but it is definitely more than 20.
