Equivocarse is not reflexive, but pronominal. Since there can be only one "se", and this is part of the verb, you cannot add another one to make the sentence impersonal (or passive), so you have to use a different approach:

At times one makes mistakes = A veces uno se equivoca / comete equivocaciones/errores...
