A scream is usually higher pitched and you have less control of your pronunciation than when you shout. A scream would be accompanied by a nuance of a high state of emotion, fear, anger, frustration, pain, etc.

A shout could simply be a raised voice with no emotional context, but they can be synonymous depending on the situation.

Following on with Pesta's answer.

"Gritar" is the infinitive for shout, and I find in RAE "Chillón" for scream.

I would add... A scream usually carries no words, and a shout is usually a communicated word or phrase.

I started to say that, but then I thought about screaming for help or screaming "Fire" and thought better of it. - 0074b507, May 10, 2011
