Only one - Spain. The rest of us just speak some weird incomprehensible lingo that we like to call Spanish, just to feel a bit better about ourselves.
You know, much the same thing that happens with English.

Well...I don't want to be too technical but I am almost 100% sure that there is a spanish speaking person in almost every country. If you mean how many countries have a national language of español, then it would be about 21. There are many people who will argue some places are "unoficial" countries so its very debate-able. Sorry for the bad spelling-in a rush!
