When does one use "Hace que"

Question

In the sentence: Cuanto hace que ha estado tosiendo?

I am assuming this translates to: How long have you been coughing?

Can tiempo be substituted for hace que in this sentence? If not, why?

You could add tiempo:

¿Cuánto tiempo hace que....?

¿Hace cuánto tiempo que....?

Not much difference than from English where you can say "For how long...?" or "For how long a time....?"

I''m not so sure that your question couldn't be asked using the present tense of toser and still be translated as "have been coughing".

¿Cuánto tiempo hace que toses?

¿Cuánto hace que estás tosiendo?
