If I were to say, "It has been five years since I have played the piano,"
What tense do I put tocar in if I use "hace tres anos que" ?
Ex. hace tres anos que toco el piano.

would you not use "Hace tres años que no tocaba el piano."?
You might want to read this to note the difference in meaning between using the present and past tense after the time phrase.
Note the differences and similarities:
Hace un año que estudio español...... I have been studying Spanish for one year.
Hace un año que estudié español....... I studied Spanish one year ago.
Estudié español hace un año....... I studied Spanish one year ago.
