I am fairly new myself here, but I know people are going to suggest you try to look up the words yourself, first. There are great facilitators on this site in the form of the translation engines and the dictionary.

I would type in:

Health Maintenance organization

Preferred Provider organization

Association health care savings plans

and see if you can't cobble together the titles you need. If you are still having a problem, write back with the exact issue (word, sentence, etc.) that is stumping you and I'm sure one of the contributors will be happy to assist.
