joeshawn has a questionnaire that she would like proofread. Any comment would be appreciated (usted vs. tú, etc.) Additional options/question, etc. I only posted this to help her with formatting. (don't be concerned about lack of check boxes, numbering, etc. That will be added outside of this editor.

(Ojo, porque lo de ¿"comes con regularidad"? es una pregunta un poco rara: en realidad, todo el mundo come con regularidad, es decir, cada cierto tiempo; el que no come con regularidad se muere. Quizás sea más acertado preguntar si uno mantiene el horario a la hora de alimentarse)

Concuerdo con Agora--así como eliminamos "enfermizo" por ser un poco fuerte, también deberíamos evitar preguntas que implican un juicio de valor. La pregunta la que sugiere Agora se mantiene neutral, como las demás preguntas de este cuestionario. - hhmdirocco, Mar 4, 2011

De igual manera, basta con "muchos dulces" y "mucha carne", porque no implican ningún juicio. Lo que es más, el estándar de uno en cuanto a "demasiados dulces" o "bastante carne" puede ser muy distinto al de otro. - hhmdirocco, Mar 4, 2011

La diferencia entre "tomar" y "comer" puede ser interesante.

Mientras en ciertas regiones del mundo donde se habla español en el lenguaje cotidiano se usa "comer verduras", en otras se emplea la expresión "tomar verduras". En México el verbo "tomar", se utiliza normalmente en el sentido de "beber" referido a la ingestión de alimentos.

Habrá que tomar en consideración el lenguaje de los encuestados para optar por el empleo de uno u otro verbo.

Under the headline "¿Con qué frecuencia comes verduras?" one of the choices is "Entre uno y dos comidas." Is this per day? Por día? I would clarify this. Also near the bottom it says "Como poco fruta". I would change it to Como poca fruta. Just above that it says "Bebo poco agua." Agua is feminine but I'm not sure if we want to say poco or poca here since agua starts with an a. Maybe somebody could clarify this.

Good point--that does need to be clarified ... al día, or, por día. Since "agua" is a feminine noun, all adjectives modifying it must be feminine. However, it takes the definite article "el" to avoid the one-word sound of "l'agua." - hhmdirocco, Mar 2, 2011

It is an adjective describing how much water is drunk. See comment under my answer. The unhealthy behavior here is not drinking little (adv.), but drinking little WATER (adj.). Drinking little is a healthy behavior when it comes to soft drinks, for ex. - hhmdirocco, Mar 2, 2011

Poco describes the amount of water that is drank and not the manner in which it is drank so I would say it is an adjective - fatchocobo, Mar 2, 2011

Going along with the consensus and saying poca is an adjective here, it would be poca agua because the "a" in poca is not stressed. It is the two stressed 'a's in la agua that are hard to discern. - 0074b507, Mar 2, 2011

The English would help to be sure about a couple phrases.

If the first question is, "How many meals a day do you eat?" or "How many times a day do you eat?" (one of these seems to be indicated by your translation and the answers), I would say, "¿Cuántas veces comes al día (or, diariamente)?".

If the question is, "How much do you eat a day?", I would say, "¿Cuánto comes diariamente?".

I'm not sure what the last question would be in English, but it doesn't sound right. Would it be something along the lines of "What things do you do that negatively affect your health?"?

Just glancing over it, I think you did a great job. Maybe a native speaker could perfect it, if need be.

As far as the tú/Ud. question, I would do it in the formal, as most professional documents are done in that format in most places; but this could vary regionally or according to use (for example, an informal survey within a group or club). - hhmdirocco, Mar 2, 2011

It is an adjective defining "how much" (not "how big") fruit/water. It is not describing the manner of the drinking (adverb). If the noun weren't there, it would be an adverb ... "Como poco." ... "Bebo poco." - hhmdirocco, Mar 2, 2011
