It is: ¿Sara va a la escuela? Si, va cuatro veces por semana

or Si, ella va cuatro veces por semana/a la semana.

in this case it is ok to skip the ELLA because you´re already talking about her in the question. it is implied in the VA.

Spanish doesn't use the verb "hacer/to do" in the same way that we do in English. So "hacer" is not necessary in the sentence.

In a question you would more likely say ¿Va Sara a la escuela? but to be clearer, you might say ¿Asiste Sara . . .? if you actually mean does she attend school. "Va" could also be interpreted as "Is she going" as in right now.
