How do I say "caregiver", as in someone who takes care of children or the elderly?

I am leading a discussion group for parents of young children. There is a survey they need to fill out, and the first question is: "I am the parent/caregiver of a ..." I need to translate the survey into Spanish, and I would like to know how to express "caregiver" correctly in the context.
