How do I fill out a Spanish airplane ticket

I have to fill out a fake airplane ticket to Spain for my spanish class...and I need help I need help on how exactly to fill it out. It looks exactly like an actual airplane ticket.

Airline tickets are standard worldwide. Do it as you normally would but in Spanish and voilá.
