I want to say: ¿es este el camino a casa?.
I think it could be this sentence but I am not sure.

"Is this the way home?" (¿es este el camino a casa?)
"Is this the way to my house?" Is this possible too?

Yes. "¿es éste...?" Not "to house" though. Either "to the house" or "to home", if it's specifically "my / your house", el hogar.
