my wife has a thyroid problem and needs synthroid. How would I ask for this at the pharmacy?

Above is the Wikipedia Spanish link for the medication. It lists a number of potential commercial names, I would suspect it varies depending on what part of the world you are looking for it in.

It includes this statement:

La UE ha estandarizado recientemente el uso del nombre "levotiroxina" para la droga. Las marcas comerciales comunes incluyen en Europa a "Thyrax", "Euthyrox", "Levaxin", "Eltroxin"; en EE. UU. "Levoxyl" y "Synthroid".

Synthroid is a brand name. I would guess the pharmacist would know it by that name.

"levotrinoxina" seems to be the translation " but you can confirm it in english and spanish here:

I believe you should hand the doctor's prescription to the pharmacist and let them handle it. Synthroid is a trade name.
