Two verbs in a sentance...

If I am trying to say, "Last week, I woke up early to exercise at the gym." - I know woke up should be desperte' - Do I also have to make the word "exercise" in the past tense as well?

Correct me if I am on the wrong track, but seeing how you don't use the past tense in English, the rule would follow in translation to Spanish.... to exercise = hacer ejercicio smile

No, the second one should be left in infinitive.

Lazarus is right: the second one should be left in infinitive:

La semana pasada me desperté (o levanté) temprano... (in past tense)

...para hacer ejercicio. (preposition + infinitive)

...para ir a correr.

...para ver el noticiario.

etc.
