Am I missing something or is that "Say how you feel when I think about you?" How does that make sense?

Actually, it says "Say that you feel when I think about you", since "qué" doesn't have an accent, and therefore is pronounced unstressed. Most likely it should be written with an accent, meaning "what".
Now, if you know something, you ask "what do you think?"; if you say something, you ask "What do you say?", so if you feel something (transitive), then in theory, you should ask "What do you feel?", and yet, it is more common to say "How do you feel?" instead, probably because you can also "feel well" (intransitive), where "well" is not an object. "What" asks for an object, while "How" asks for an adverb of manner, not an object. "Di qué sientes" is asking for a noun or action that describes the feeling (a direct object), not the manner in which you feel.
Tell me what you feel when I think of you time and time again. Each moment you are not with me my world is upside down. I walk on a desert when you are gone. I am not sure if you are a mirage since you seem so real.
