Cómo has hecho eso = cómo lo has hecho eso = cómo hiciste eso = cómo lo hiciste eso.
It's wierd how in Spanish have is like that. ¿Fue bien? = ¿Ha sido bien? = Was it good?

¿Cómo has hecho eso? ¿Cómo hiciste eso? are perfect.
You don't need lo if you have eso in the same sentence. (Cannot have two objects for the same verb). If you say ¿Cómo lo has hecho eso? it would be as saying How have you done it this? So, please just say ¿Cómo lo has hecho? or ¿Cómo lo hiciste?
Was it good? is usually translated as ¿Estuvo bien? because the result is unpredictable, so using Estar takes care of this transient condition, which cannot be expressed with Ser.
