Yes, in "Usted" form, only put the tilde on "Está" and "Cómo".
- In "Tú" form it's Hola ¿Cómo estás?.

yes that is the formal version. thats how you would say it to someone you just met. to your friends or famly you could also say " hola como estas" thats the informal way to say hello how are you.
