Tengo algo en que pensar. ('I have something to think about' or, more literally, 'I have something about which to think.')

No entiendo lo que me estás diciendo. ("I don't understand what you are telling me.")

No entiendo de que hablas. ("I don't understand what you are talking about")

They are grammatically quite different. "Que" and "lo que" are relative pronouns, "de que" is a conjunction, and "en que" is part of a prepositional phrase.
