How to begin forming sentences?
I have alot of basic words and phrases down, but when it comes to the point where I have to come up with my own sentences, I always choke up. I need to know which type of words go before which. (nouns before verbs, etc)

The order of the words is generally the same as in English, with the notable exception of the "experiencing" constructions required with verbs like "gustar". In any case, you'll need to read, listen and imitate a lot before you can produce perfect sentences with ease.
One approach used in california schools is to learn "formulas" then you input vocabulary. For example you learn the phrase "Me gusta _____" Then you input the vocabulary. It's simple, not what natives are doing most of the time but it gets you started. Then you modify that formula, "Me gusta ____ _____" as in "Me gusta mucho el chocolate." And you keep building. Eventually you start creating things outside of the formulas and that is a big step, when it happens you will make lots of mistakes but that is part of learning and if you don't make mistakes you won't learn as fast/well. The short and long of it, if you want to start speaking in sentences, just speak. Natives tend to be supportive and understanding to people who are really trying.
