Se me olvidó hacere mi tarea. - Se me olvida hacer mi tarea. The former for a specific occasion, and the latter for a general tendency. Neither is likely to win much sympathy from your teacher.

"I forgot to do my homework." (preterito)

Or, another good excuse: "My dog ate my homework."

If you are looking for a sentence that uses 'forget', then...

"I cannot remember your name, but I never forget a face."

No puedo recordar tu nombre, pero no me olvidé una cara.
