Hmmm not any easy one to describe but I would say 'ache' is more ongoing, over a long period of time where as 'hurt' is more present and possibly resulting from a specific injury etc
My stomach aches. eg it's been sore all day
My stomach hurts. eg Johnny just threw the ball and it hit me in the stomach
I can't guarantee any grammatical rule behind this but rather just my perception. 
ACHE = dull, constant, ongoing pain
HURT = sharp pain, achingpain
If you say for example, "My knee aches," I would think that the pain doesn't result from any particular recent injury or event, but is (as zemirah wrote) an ongoing, probably dull state of pain, like from arthritis. For example, "My knee hurts" would tend to make me think it's a sharper pain, and maybe (though not necessarily) the result of something specific that happened. But we're talking about nuances here. 
Ache is usually used with a part of body to form a compound word like heartache, toothache, backache, headache... Pain is simply used to describe what HURTS us. Hurt can be used as a verb, adjective, noun in general and abstract noun especially in songs and novels, love stories, for ex.

"I have toothache/earache/backache."
"He was hurt when he fell down the stairs."
"I feel pain when I raise my arms above my head."
I think the thing you should do is type them both into the dictionary and see what comes up.
Ache is more common as a noun, and hurt is more common as a verb.
An ache is a dull pain. It is never a sharp, stabbing pain.
There are some terms and phrases that use one exclusively:
toothache headache heartache aches and pains (generally, arthritis) My heart aches for you. (This is an example of ache as a verb). My joints ache. My head aches.
Are you hurt? Who hurt you? I am going to hurt you. I am hurting.
You could say, "My toothache really hurts."
