Welcome to the forum Andre. I'm just a beginner here so maybe someone will come and correct me but here is what I would say: "Me gustaría hacer una cita..." and the rest you can add to for context. Of course noboby gets anything for nothing here so you should take it to the translater and see what it actually says. You can even try that with your question.
