You could ask something like "¿Puede deletrear su nombre y apellido para mí?

Just ask them slowly and loudly in English. Repeat, more loudly each time until you are understood...

How about, 'please can you spell your name for me? I need to put it on your wristband'? You can put this into the translator and translate it, see what you get and post your translation and we can help you correct it.
