Si quiero decir "how sad!" en espanol, debo decir "que triste" con acento sobre la "e" o sin acento? gracias!

Qué = what
Que = that, how

Qué con tilde se usa en las interrogaciones y en las exclamaciones Por ejemplo: ¿Qué vas a hacer mañana? ¡Qué hiciste, está todo mal!
También se usa en las frases que sin tener interrogaciones, son preguntas. Por ejemplo ...Y entonces le preguntó sobre lo qué hizo.
En el resto de los casos no lleva tilde
