'Student finance' y 'customer reference number' en espanol + uso de usted

Question

Are these translations correct:

Student Finance Spain - Finanzas Estudiantes España (I don't think this exists, but we have Student Finance England here)

Customer reference number - número de referencia de cliente

Is this use of the formal usted right?

Por favor, escoja usted una de las opciones siguientes Sí ya ha aplicado..... Pulse usted uno ¿En qué puedo ayudarle?

First of all, I´d like to tell you that aplicado for applied is wrong,a widely spread wrong use of a false friend.To apply is hacer una solicitud

Customer reference number - número de referencia de cliente is okey. I would simply say Número de cliente.
