I volunteer in a free clinic that provides primary medical care to non-insured patients. My job is to collect and process laboratory samples, mostly blood and urine. We have someone at the clinic that worked in Peru for a couple of years, but some of the dialects vary from country to country. We need to keep the discussion professional, so slang is not proper.

Here are the sentences I've developed. Could someone please review and make recommendations for the best way to ask or address patients from all over latin america?

¿Cuál es su nombre? - what is your name?

¿Cuando es tu cumpleaños? - when is your birthday?

Tengo que tomar una muestra de sangre to test. - I need to take a sample of blood para la preuba.

I have not been able to develop a good method to tell the patient I need a urine sample. It should probably sound something like

Tengo que una muestra de _______ para la preuba.

Well, I agree with the sugestions already made, so I would say the following:

1- ¿Cómo se llama? ("¿Cuál es su nombre?" is also fine, but in my opinion this sounds more natural, it can differ depend on the country).

2- ¿Cuál es su fecha de nacimiento?

3- Tengo que tomarle una muestra de sangre para analizarla

4- Necesito una muestra de orina para analizarla

Necesito una muestra de orina para analizarla or Tengo que pedirle una muestra de orina para analizarla.

"para la prueba" is not wrong, but "para analizarla" sounds more professional.

You don't ask for "cumpleaños" on a form. The usual way is to ask for "fecha de nacimiento".
