[urgent] Writing an address in Spanish

Hello folks, I have problem in filling out the Spanish form. Can someone give me a hand? I suppose the "direccion" is for writing the whole address. However, the items follow "direccion" confuse me. I have no idea what to write for the Calle, numero, and piso. Beside, I have no idea why I should write the same thing again as I have already written the information in the "direccion" column. Calle = street
Numero = number
Piso = floor
