The subjunctive is required in this case. Remember that one of the uses of the subjunctive is to tell someone to do something or to recommend that someone do something. Therefore, the sentence "I suggest that you go" will be: "Sugiero que te vayas" Similarly, "I recommend that you take medicine" will be: "(Yo) recomiendo que tomes la medicina" or in this case you can simply say "(Yo) te recomiendo tomar la medicina" Either is correct
Ok, the way a native speaker would say that would be "Te recomiendo que tomes la medicina" or simply "Recomiendo que tomes la medicina". "Te recomiendo tomar la medicina" is fine too, by the way.
"que tomar" is just wrong. The subject pronoun at the beginning is redundant and wrong, unless there's more people giving recommendations to the same person.
Also, that verb in subjunctive mood, "tomes", isn't there because it is "future tense". This can be interpreted as a recommendation to take the medicine tomorrow or right now. It is subjunctive because whenever you give an order, recommendation, etc.. inside a d.o. ('que..') rather than with a verb in imperative mood, it needs to be in subjunctive. In fact you could simply say "Toma la medicina" to say the same thing, but more emphatically.
Finally, you can't say "Te recomiendo que tomas", that is just as wrong as 'que tomar'.

I would actually make it reflexive, and say "Yo te recomiendo que tomar la medicina."

I use tomes to indicate take(future tense), tomas=are taking.
why tomes and not tomas?
