This can depend on the context. If someone is bowing down to God, as the Bible verse Genesis 17:3, the translation was "cayó rostro en tierra" (NVI).

The other translators on this site offer

postró sobre su rostro

and

se cayó en la cara

The translator works pretty well on this one. grin

As you should be able to tell from the replies, the meaning of "fell on his face" is subject to interpretation. At the very least "he went from a standing position to a prone (face downmost) position" or "he made a fool of himself". To echo Keown, "context".
