The guy on the right is "perro". The guy on the left is "perrito".

perrito would be with a smaller dog.
the -ito ending usually signifies a smaller version of something

perrito can be used as a term of endearment for a dog that you like whole lot. Oh you sweet perrito, Come give me a kiss.

I thought that el perro is a grown (adult) dog and a perrito was a puppy.

It depends on where you are. Around here (SoCal) perrito = puppy. Cachorro can be a young mammal of many sorts. In other places cachorro is puppy and perrito is a little dog. - KevinB, Oct 20, 2010
