I sent a form to this man in Mexico and asked him to review the answers to see if all was correct.
This was his answer:
nada mas falto mi numero de celular no esta hay por alguna emergencia es 55555555555 ok y lo de mas esta todo bien
Google translates that as:
nothing but miss my cell phone number is not 55555555555 for any emergency is ok and what's more it's all right

Only my cellphone number was missing, for any emergency it's 555555555... everything else is all right.
