Are the above sentences correct? Does one way sound better than the other?
Are both "olvidé" and "se me olvidó" acceptable for "I forgot"? Does one sound more natural than the other?
After the phrase "me habías pedido que" would subjunctive be used? If so, why?

Regarding your sentence, I would say:
Me habías pedido que te lo enviara hace un mes y lo olvidé or,
Me habías pedido que te lo enviara hace un mes y se me olvidó.
Me había pedido a que lo mandara hace un mes, y se me olvidé!
You asked me to send it to you a month ago, and I forgot!
This is the way I would say it.
