You had asked me to send it to you a month ago and I forgot.

Me habías pedido mandartelo hace un mes y olvidé.

Me habías pedido mandartelo hace un mes y se me olvidó.

Me habías pedido que te lo mande hace un mes y olvidé.

Are the above sentences correct? Does one way sound better than the other?

Are both "olvidé" and "se me olvidó" acceptable for "I forgot"? Does one sound more natural than the other?

After the phrase "me habías pedido que" would subjunctive be used? If so, why?

Regarding your sentence, I would say:

Me habías pedido que te lo enviara hace un mes y lo olvidé or,

Me habías pedido que te lo enviara hace un mes y se me olvidó.
